How to Place a PMG Wholesale Order: 4 Easy Steps

Step 1: Choose the Teeth

Use our tooth chart (attached below) to help your client select which teeth they want their grill on.

Step 2: Fill Out the Order Form

Submit the order using our JotForm

If you’re using a 3D scan, you can upload the STL files directly through the form.

 

Step 3: Pay the Invoice

Once the form is submitted, an invoice will be sent to you. Please complete payment before moving forward.

Step 4: Send the Dental Impression

Mail the client’s dental impression to the shipping address provided after payment.
